Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. a. In the nutrition/beverage area where employees were setting up coolers, the cord on the vacuum pump was pulled away from the motor with interior wiring showing and rubbed through to the copper conductor, thereby exposing the employees to a 112 volt contact hazard.

29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired. a. In the nutrition/beverage area where employees were setting up coolers, the cord on the vacuum pump was pulled out of the motor exposing the copper conductors of the wires and had not been inspected for damage prior to use, thereby exposing the employees to a 112 volt contact hazard. b. In the nutrition/beverage area where employees were setting up coolers, the extension cord being used to provide power to the vacuum pump was damaged in two areas and had not been inspected for damage prior to use, thereby exposing employees to a 112 volt contact hazard.
