29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: (a) On or about 11/21/2013 - 720 Gallatin Street, Huntsville, AL, the employer allowed debris such as, but not limited to, boards with nails, metal studs, sections of pipe, wire rope, concrete waste, bags, bottles, cans, food wrappers and straps to accumulate in the work areas creating slipping, tripping, and fall hazards.

29 CFR 1926.501(b)(4)(iii): Each employee on a walking/working surface was not protected from objects falling through holes (including skylights) by covers: (a) On or about 11/21/2013 - 720 Gallatin Street, Huntsville, AL, holes on the 2nd and 3rd floors where the air ducts pass through the floors were not guarded in a manner to keep materials, tools, and trash from falling through and striking employees working below.
