29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section.    Location -  149 County Road, Ipswich, MA:  On or about 11/19/2013, an employee was not protected from trench cave in or collapse while taking measurements in a trench that was in excess of five feet in depth.
