29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas are being used on the jobsite: (a) 214 Elm St West Springfield, MA : On or about November 12, 2013, employees working at this job site had more than 5 gallons of gasoline (to power their aircompressor/generator), and no fire extinguisher as required by this subpart.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) 214 Elm St West Springfield, MA: On or about November 12, 2013, two employees were working on the roof of this building more than 19 FT (lower roof) above the ground below without any fall protection as required by this subpart: Independent Roofing Co., Inc. was previously cited for a violation of an equivalent standard 29 CFR 1926.501(b)13, which was contained in OSHA inspection number 313201444, citation number 1, item number 1 and was affirmed as a final order on September 22, 2009 with respect to a workplace located on Southwick Rd in Westfield, MA: Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
