29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a. Site - Employees were not protected from falls of approximately 20 feet while working from the attic decking when fall protection was not used or provided.     b. Second floor - Employees were not protected from falls of approximately 12 feet while performing carpentry work when guardrails were not installed in the window openings.

Site - Employees were not protected from falls of approximately 10 feet when a Werner fiberglass step ladder was used to gain access to and from the attic decking and the side rails only extended approximately 6 inches above the upper landing surface.

Site - Employees were exposed to tripping hazards when a Werner fiberglass step ladder was positioned with its base against a stack of plywood.     b. Site - Employees were not protected from falls of approximately 10 feet when a Werner fiberglass step ladder was positioned between two trusses in a way which created an unavoidable obstacle at the top of the ladder to employees ascending and descending.
