29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. At the location: Optometry Clinic at 1800 South McCall Road, Englewood, FL 34223 (a) for the two employees working on the entry cover of the front of the building, north side at approximately 14 feet and 4 inches high, as observed on or about 11/19/2013. Employees were observed using the scaffold end frames as a means of egress and access.

(b) Two employees were working on the entry cover of the front of the building, north side at approximately 14 feet and 4 inches high without fall protection equipment being installed or utilized, observed on or about 11/19/2013.
