29 CFR 1910.177(c)(1): The employer did not provide a program to train all employees who service rim wheels in the hazards involved and the safety procedures to follow while servicing rim wheels: a) On or about November 22, 2013 the employer did not assure that employees were protected from struck-by hazards when installing and inflating tires onto single piece rims. The employer had two tire cages onsite but they were not used when necessary.
