29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points were not protected from damage:    (a) On or about November 15, 2013 MSB Construction, Inc. ran a drop cord through a metal doorway and did not protect the cord from the pinch point damage.    (b) On or about November 15, 2013 MSB Construction, Inc. ran a drop cord across a driveway where the cord was subject to being pinched as vehicles traveled over the cord.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    (a)  On or about November 15, 2013 a worker was in a trench 10'-1" deep and was not provided a ladder for egress from the trench.

29 CFR 1926.651(k)(2): Where the competent person found evidence of a situation that could result in possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employee(s) was/were not removed from the hazardous area until the necessary precautions have been taken to ensure their safety:    (a) On or about November 15, 2013 a worker in a trench was not removed from the trench when unsafe conditions were present including the trench having vertical walls and not being shored or otherwise protected from collapse and where a safe means of egress was not present.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) or otherwise protected the trench wall from failing and collapsing into the trench.    (a) On or about November 15, 2013 MSB Construcion Inc. had a worker in an excavation 10'-1" deep.  The excavation walls were vertical (90 degrees from horizontal) and the excvation wall consisted of fill dirt and gravel, and water had been present in the bottom of the trench.
