29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a. Where equipment such as the teardown machine, plug cutter, and table saw were used, a lockout program, according to the requirements of the standard, was not developed and implemented, including writing specific machine lockout procedures, documented training for employees performing lockout work (such as changing saw blade) or affected employees working on machines, and requiring lockout of machines during repair or maintenance.  Abatement documentation is required for this violation.

At the teardown machine, outer insulation was damaged on the conductors to the machine disconnect;  b. At the teardown machine, conduit was separated on the line to the machine. On the east side of the building where pallets were assembled, the ground fault circuit interrupter protection was not functioning on the 110 V outlet used for powering a vehicle motor heater, and available for other use.
