29 CFR 1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective. 1. Inside of the employee trailer, employees were using a white Westinghouse microwave that was not grounded. The metal casing (framing) of the appliance was energized. The ground pin was missing from the plug of the electrical cord used to energize the appliance.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed. 1. Inside of the employee trailer, there was a 120/240 volt electrical panel box with four (4) unused openings located inside of the breaker box. The openings were covered with black tape and employees were exposed to physical contact with live parts.

29 CFR 1910.305(b)(2): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: In completed installations each outlet box shall have a cover, faceplate, or fixture canopy: 1. Inside of the employee trailer, there was a cover missing from the breaker box used to provide electricity to the trailer. The breaker box was located on the west wall of the trailer and employees could make physical contact with breakers and live parts in the box because no cover was in place.
