29 CFR 1910.101(b):  Section 3.4.4, of the Compressed Gas Association Pamphlet P-1 1965,  as adopted by 29 CFR 1910.101(b):  Compressed gas cylinder(s) were not properly supported  to prevent them from being knocked over:         a) On or about November 14, 2013, at the above located workplace, in the slop area, three compressed gas cylinders were stored without chains or other means to prevent falling. Employees were thereby exposed to the hazards associated with falling compressed gas cylinders.      b) On or about November 14, 2013, at the above located workplace, in the purchasing area, a compressed gas cylinder was stored without a chain or other means to prevent falling. Employees were thereby exposed to the hazards associated with falling compressed gas cylinders.

The path to ground from circuits, equipment, and enclosures was not  permanent, continuous, and effective.  a) On or about November 14, 2013, at the above located work place, employees worked with and around the following equipment which was not properly grounded:       a) Two refrigerators (One Sanyo and One Hilton) in maintenance shop.       b)  One portable power strip.        c) One General Electric freezer in the kitchen.        d) One Wind Handler floor fan/blower in the housekeeping equipment area.       e) Two Titian shop vacs in the housekeeping equipment area.        f) One Betco floor buffer in the housekeeping equipment area.    The employees the equipment were thereby exposed to an electrical hazard.

29 CFR 1910.305(j)(1)(i): Fixtures, lampholders, lamps, rosettes, and receptacles had live parts exposed to employee contact:         a) On or about November 14, 2013 at the above located workplace, employees in the kitchen worked  with and around a damaged 120 volt outlet with exposed wires. The outlet was located to the right and across the passageway from the  eyewash station. The wiring was not enclosed in an approved box or other approved enclosure. The employees were exposed to energized electrical wires and to the hazards associated with energized parts.   b) On or about November 14, 2013 at the above located workplace, employees in the kitchen worked with and around a damaged 120 volt outlet with exposed parts. The outlet was located to just to the left of the eyewash station. The exposed energized parts were not enclosed in an approved box or other approved enclosure. The employees were exposed to energized electrical parts and to the hazards associated with energized parts.

29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not installed so as to prevent moisture or water from entering and accumulating within the enclosure.  In wet locations the enclosure was not weatherproof.         (a)  On or about November 14, 2013, at the above located workplace, employees worked with and around a 120 volt junction box in a wet location under a kitchen sink. The box was loose and had exposed energized wires. Employees that worked at or around the sink were thereby exposed to an electrical hazard.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:                 a) On or about November 14, 2013, at the above located workplace, in the kitchen, employees regularly worked with Triple F, Oven Brite Concentrated Oven Cleaner and other corrosive cleaning chemicals. The eyewash spray head for the nearby eyewash was detached and rested next the eyewash. The detached spray head required extra steps to get the eyewash to work properly (finding and attaching the head), this increased the time required to get water to the eyes.
