29 CFR 1926.405(b)(2): In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy:     (a) Jobsite, Consad Bldg., 2nd Floor, 123 N. Highland Avenue, Pittsburgh, PA:  On or about 11/22/13 the outlet boxes used to provide temporary power did not have covers in place.   A flexible cord was plugged into one of the outlets and was being used to supply power to a hand mixer.
