29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:  (a) work bench on south side, new residential construction located at 31 Whitlock Drive SW, Marietta, GA 30064: Employee was exposed to severe laceration and amputation hazards while operating two Makita (Model 5007F) 7 1/4-inch Circular Saws without a lower guard as the guard was shimmed to prevent the lower guard from retracting after a cut was made by the operator.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end.  (a) South side, new residential construction located at 31 Whitlock Drive SW, Marietta, GA 30064: Employees working on a pump jack scaffold were exposed to falls of approximately 12-feet, 9-inches to the lower level as the scaffold plank extended approximately 7 feet past the supports without the cantilevered portion secured to the pump jacks with the chain provided to prevent tipping while in use.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  (a) South side, new residential construction located at 31 Whitlock Drive SW, Marietta, GA 30064: Employees were exposed to falls of approximately 12-feet, 9-inches while installing Hardie-Plank Lap Siding from a pump jack scaffold which was not erected on a firm foundation as the scaffold supports were sinking into the soft soil exposing employees to possible scaffold collapse.

(a) South side, new residential construction located at 31 Whitlock Drive SW, Marietta, GA 30064: Employees were exposed to possible collapse of the pump jack scaffold system as the support poles were not secured at the bottom of the supports with triangular bracing or equivalent to keep the poles plumb and prevent accidental displacement while in use.
