29 CFR 1910.29(a)(3)(ii): The minimum platform width for any work level was less than 20 inches for mobile scaffolds (towers). Ladder stands shall have a minimum step width of 16 inches. On or about November 5, 2013 warehouse worker was standing on the top handrails of a rolling ladder, model H-1083-10, to access top shelves to retrieve a plastic part. The handrails were approximately 1 inch in diameter. This hazard was located in the warehouse and exposed employee to a fall from 8 feet 6 inches of elevation landing on concrete floor hazard.

29 CFR 1910.178(m)(3): Unauthorized personnel were permitted to ride on industrial truck: On or about October 5, 2013 an employee working in the warehouse was standing on top of the forks of a Doosen, Model G30E-5, forklift to load empty cardboard boxes, exposing him to a fall from elevation hazard of approximately 10 feet to a concrete floor.
