29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. (a) Upper Level (Main Dock) - The employer did not ensure that the area was kept clean and in an orderly state. Boxes were stacked too high and stored on the floor throughout the area exposing employees to struck by and tripping hazards. (b) Upper Level (Cosmetic/Gift w Purchase Area) - The employer did not ensure that the area was kept clean and in an orderly state. Boxes were stacked high and stored on the floor exposing employees to tripping hazards. (c) Upper Level (Bailer) - The employer did not ensure that the area in front of the bailer was kept clean and in an orderly state. A skid containing a bail and empty boxes were observed stacked near and in front of the bailer exposing employees to tripping hazards. (d) Basement - The employer did not ensure that the area was kept clean and in an orderly state. Boxes were stacked on top of each other and on the floor near the conveyor machine exposing employees to struck by and tripping hazards. (e) Basement (Maintenance Storage Area)- The employer did not ensure that items stored in this room were stored in an orderly state. Boxes were observed to be on top of each other on the floor throughout the room exposing employees to tripping hazards. (f) Penthouse - The employer did not ensure that the area was kept clean and in an orderly state. Seasonal and display items were stored in a manner where they were extending onto the aisles at eye level and foot level, exposing them to struck by and slipping hazards.

29 CFR 1910.22(b)(2): Permanent aisles and passageways were not appropriately marked: (a) Upper Level (Main Dock) - The employer did not mark aisles and passageways permanently to limit employees from storing items anywhere throughout the floor when unloading trailers. Boxes were stored in front of the dock area, the fire exit and bailer. (b) Basement - The employer did not mark aisles and passageways permanently to limit employees from storing items anywhere throuhgout the floor when items are coming off the conveyor system. Boxes were scattered in front of and around the conveyor system.
