Abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:    (a) 504 Lakeside Dr, Lee's Summit, MO 64064; Scaffolding used by employees applying (EIFS) External Insulation and Finishing System was not equipped with base plates, mud sills or other adequate firm foundation. The scaffold rested on wood blocks on the driveway and was cribbed with wood on the mud areas near the house front door. Employees were exposed to potential scaffold collapse hazards.    Abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    (a) 504 Lakeside Dr, Lee's Summit, MO 64064; No access ladder was used to access tubular welded scaffolds. Employees were exposed to fall hazards when climbing crossbraces and end panels of the scaffolds. End panels were not configured with uniform rung-like supports.    Abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. No fall protection was worn and no adequate guard rail system was in place.
