29 CFR 1926.452(k)(2): Ladders used to support the ladderjack scaffolding did not meet the requirements of Sub-part X:      a)  On or about November 22, 2013 and at times prior thereto at the jobsite; ladders used to support the ladderjack scaffold didn't meet Sub-part X requirements in that employees were using 2 sections of an extension ladder seperately, both sections were used at an angle greater than a quarter of the ladders length, and one of the ladder sections was placed on a slippery surface in the trash container.
