Graber Construction, Incorporated worksite located at 14322 CR 15, Holiday City, Ohio: On or about November 8, 2013, the employer did not ensure the employees personal fall arrest system was secured (clipped-off) to the Aerial Lift. The exposed employees were installing perlins on the South end of the structure near the peak which was approximately 60.00 feet.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. A. D.C. Graber Construction, Incorporated worksite located at 14322 CR 15, Holiday City, Ohio: On or about November 8, 2013, the employer did not ensure employees were protected from falls while applying new roofing material to a low sloped commercial structure. Each employee engaged in this activity was exposed to a potential fall distance of approximately 24.00 feet on the East and West side of the structure.
