29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :     Annual audiometric testing was not provided within twelve months of the date of the previous audiometric testing for the employees with the following exposures:     a)   Bench area - A stone finisher was exposed to continuous noise levels in excess of the allowable 8 hour time-weighted average sound level (Permissible Level). The employees dosimeter dBA level was 185.8% of the permissible level (90 dBA) and was equivalent to approximately 94.5 dBA. The sampling was performed for 480 minutes during one shift on 12/4/13. Zero exposure was assumed for the unsampled period of time.      b)   Bench area - A stone finisher was exposed to continuous noise levels in excess of the allowable 8 hour time-weighted average sound level (Permissible Level). The employees dosimeter dBA level was 178.6% of the permissible level (90 dBA) and was equivalent to approximately 94.2 dBA. The sampling was performed for 480 minutes during one shift on 12/4/13. Zero exposure was assumed for the unsampled period of time.    c)   Bench area - A stone finisher was exposed to continuous noise levels in excess of the allowable 8 hour time-weighted average sound level (Permissible Level). The employees dosimeter dBA level was 133.5% of the permissible level (90 dBA) and was equivalent to approximately 92.1 dBA. The sampling was performed for 450 minutes during one shift on 12/4/13. Zero exposure was assumed for the unsampled period of time.    d)   Bench area - A stone finisher was exposed to continuous noise levels in excess of the allowable 8 hour time-weighted average sound level (Permissible Level). The employees dosimeter dBA level was 119.4% of the permissible level (90 dBA) and was equivalent to approximately 91.3 dBA. The sampling was performed for 480 minutes during one shift on 12/4/13. Zero exposure was assumed for the unsampled period of time.    e)   Bench area - A stone finisher was exposed to continuous noise levels in excess of the allowable 8 hour time-weighted average sound level (Permissible Level). The employees dosimeter dBA level was 104.1% of the permissible level (90 dBA) and was equivalent to approximately 90.3 dBA. The sampling was performed for 480 minutes during one shift on 12/4/13. Zero exposure was assumed for the unsampled period of time.
