29 CFR 1926.20(b)(2):   The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person(s) designated by the employer:  a)  Jobsite located at 1201 N. 10th. Street, Millville NJ 08332:  A competent person did not conduct frequent and regular inspections of the worksite.  Company employees were exposed to hazards including but not limited to fall hazards whille performing roof removal and installation, skylight removal and hole covering and other related activities from a flat roof and less than six feet from unprotected edges without using fall protection.  Condition noted on or about 11/25/2013.

29 CFR 1926.501(b)(4): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a)  Jobsite located at 1201 N. 10th Street, Millville NJ 08332:  An employee fell approximately 15 feet to the concrete surface below while covering holes from removed skylights without any means of fall protection.  Occurred on or about 11/19/2013.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)  Jobsite located at 1201 N. 10th Street, Millville NJ 08332:  Company employees were not given a fall protection training program to enable them to recognize the hazards associated with working on unprotected roof skylight holes.  Occurred on or about 11/25/2013.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a)  Jobsite located at 1201 N. 10th Street, Millville NJ 08332:  Company employees  were exposed to fall hazards of approximately 15 feet to the ground below, when ascesing the roof surface while using an extension ladder that did not extend 3 feet above the upper landing surface to perform roof removal and installation and other related activities.  Conditions observed on or about 11/25/2013.
