It had knockouts. The receptacle was lying on the floor. It had been damaged and was bent.

29 CFR 1910.303(b)(7)(iv): There were damaged parts that adversely affected safe operation or mechanical strength of the equipment, such as parts that were broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating: a) Shop Area, east side, on the G & P Machinery Two Person Sander, the On/Off switch located at the operators working area, was broken and bypassed. The machine was turned off/on by use of the overcurrent electrical breaker.

29 CFR 1910.305(b)(1)(i): Conductors entering boxes, cabinets, or fittings were not protected from abrasions: a) Shop Area, east side, in the MIG Welder area, a 480 volt flexible cord was receiving electrical power from the electrical power switch box. The electrical wiring outer insulation was not connected to the power box, exposing the wiring to movement and damage. Additionally, strain was placed on the inner electrical connectors. b) Shop Area, Welding Station near 2 Man Sander machine, the electrical wiring conduit was not connected to the green panel control section.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: a) Shop Area, east wall: On the 480 volt yellow power cord in the MIG Welder area, and yellow flexible cord outer insulation was pulled out of its end electrical plug. It was attached to a black cord, which had its outer insulation pulled out of its end electrical plug, allowing strain to be placed on the inner electrical connections. b) New Building, east side: the 110 volt flexible cord in the Galvanized Welding area, supplying power to receptacle did not have its outer insulation attached to the frame of the receptacle, allowing strain to be placed on the inner electrical connections. c) New Building, east side: the 110 volt flexible cord in the Galvanized Welding area, the power cord to the TPI, one quarter Horse Power pedestal fan, to receptacle did not have its outer insulation attached to the frame of the fan, allowing strain to be placed on the inner electrical connections.
