29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by approved cabinets of other forms of approved enclosures, or other means listed under this provision:  a) BOILER ROOM: On or about November 25, 2013 the employer did not ensure that "live" wiring between two boilers was adequately guarded against accidental contact by an approved method.  b) BOILER ROOM: On or about November 25, 2013 the employer did not ensure that "live" wiring coming out of a rigid conduit system was adequately guarded against accidental contact by an approved method to protect employees working in the immediate area.  These conditions exposed the employees to the hazards associated with electrical shock.

29 CFR 1910.304(b)(3)(i): All 125-volt, single-phase, 15- and 20-ampere receptacles installed in wet locations were not equipped with ground-fault circuit-interrupter protection for personnel.  a) PUMP ROOM: On or about November 25, 2013 the employer did not ensure that a receptacle was equipped with a GFCI where the potential of a water leak was feasible.  b) SWIMMING POOL AREA: On or about November 25, 2013 the employer did not ensure that receptacles equipped with GFCI's were in operable condition by inspecting and/or testing them as stated by the manufacturer.  These conditions exposed the employees to the hazards associated with electrical shock.
