29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a) Job site, Route 30 near Agnew Road, Greensburg, PA: On or about November 20, 2013, during water line installation activities, the employer did not ensure that appropriate cave in protection was used in a trench/excavation determined to be up to approximately 5 foot 9 inches in depth, exposing an employee to a cave in hazard. Frank J. Zottola Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 314111493, citation number 1, item number 2, and was affirmed as a final order on January 17, 2011, with respect to a workplace located at Smallman and 14th Streets, Pittsburgh, PA. Frank J. Zottola Construction, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 312627748, citation number 1, item number 2, and was affirmed as a final order on February 26, 2009, with respect to a workplace located at 1st and Western Roads, Irwin, PA.
