Written approval from the forklift manufacturer was not obtained before Cascade clamps were installed on the following forklift trucks: - 1999 Crown SC (Model #35SCTT) - 2008 Crown RC (Model #RC5540-40TT) Condition observed on or about November 25, 2013.
