29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about November 21, 2013, at 460 Marble Falls, Grovetown, Georgia:  Two employees performing roofing work were exposed to struck-by hazards from falling hammers, nail guns, and asphalt shingles of the employees working above them when they did not use head protection.

On or about November 21, 2013, at 460 Marble Falls, Grovetown, Georgia:  Two employees performing roofing work were exposed to struck-by and impalement hazards form nails and debris when they used nail guns without wearing eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about November 21, 2013, at 460 Marble Falls, Grovetown, Georgia:  An employee performing roofing work was exposed to a fall hazard of approximately 25-feet and greater when he installed asphalt shingles on a 12/12 slope roof ridge without using fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about November 21, 2013, at 460 Marble Falls, Grovetown, Georgia:  Two employees performing roofing work were exposed to a fall hazard of approximately 11.3-feet and greater when they used an aluminum ladder to access and egress the roof without extending it three feet above the roof landing.
