29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: a) Building 12, Penthouse Unit 4, on or prior to 11/18/2013 -- unguarded rotating shaft end of the TSW Supply Fan 4-ICS did not present a smooth edge in that it was notched and projected approximately 2 1/2 inches from the expanded metal guard, exposing employees to caught-in hazards.
