29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: JOB SITE: 853 Coach Light Lane, Hazelwood, MO 63042 At this work site, workers installed new shingles within one foot of the residential electric service drop located at the northeast corner of the structure. Workers were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means. Workers were exposed to an electrical shock, burn or electrocution hazard.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: Location: 853 Coach Light Lane, Hazelwood, MO 63042 At the job site, workers were exposed to a fall hazard as they were not protected by any type of fall protection system while they actively performed roofing work on the low slope residential roof. The eave to ground height measured approximately 10 feet, 6 inches at the northeast corner and approximately 9 feet at the northwest corner where workers were shingling.

The employer allowed the workers to actively shingle on a residential low slope roof, approximately 9 feet to 10 feet, 6 inches above ground without any form of fall protection.
