29 CFR 1910.133(a)(1): Protective eye and face equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:  (a) Employees in the press area were exposed to ultra-violet light. Ultra-violet lights are present on presses to cure the print on paper as it passes underneath the light. Warning labels on the light units indicate: "DANGER" "INTENSE ULTRA-VIOLET AND VISIBLE LIGHT" "AVOID EYE AND SKIN EXPOSURE". No protective eye equipment was worn by employees working near the ultra-violet lights. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     (a) Machine guarding was not provided to protect employees from hazards created by ingoing nip points of rollers on the Didde Web Presses.     (b) Research and Development area; Bridgeport Mill, Serial Number 36638, was fitted with chuck and drill bit. The chuck spins during operation and was not guarded at the time of the inspection to protect maintenance employees from hazards created by rotating parts.     (c) On or about August 12, 2013, an employee received a hand fracture when her left hand entered the ingoing nip point between the small laminator rollers and the product being fed into the machine. Adequate method(s) of guarding was not provided to protect the operator from the hazards created by the ingoing nip point of the rollers.     (d) Adequate machine guarding was not provided to protect employees from the hazards created by the ingoing nip point of the large laminator rollers.     (e) Employees were exposed to the unguarded portion of the Work-A-Matic band saw blade. The existing guard was not adjusted low enough and did not extend up enough to cover the upper portion of the unused blade.

(b) Press Department, Didde Webb Press #8, East/gear side - Employees were exposed to electrical hazards where the ultraviolet light shutter switch was plugged into a 3 way extension, used as a substitute for fixed wiring.  (c) GYG Department, Southwest end of the center acrylic bend stations - Employees were exposed to electrical hazards where flexible cords were used as a substitute for fixed wiring.  (d) Research & Development Department - Employees were exposed to electrical hazards where re-locatable power strips, attached to vertical milling machines and the Bridgeport with the drill bit, were used as a substitute for fixed wiring.  (e) Research & Development Department and neighboring office break room- Employees were exposed to electrical hazards where flexible cords were passed through walls and were subjected to potential abrasion. One flexible cord powered a duplex receptacle and the other powered the Kenmore refrigerator in the office break room. (b) Research and Development Department - Employees were exposed to electrical hazards created by a damaged strain relief on a flexible cord located over the bench. A section of pipe or conduit covered the middle section of the flexible cord, adding weight to the wire-terminal connection.
