29 CFR 1910.179(j)(3): A complete periodic inspection of crane(s) had not been conducted in the past 12 months: Organized Maintenance Shop: On November 25, 2013, and at times prior thereto, employees were exposed to overhead and/or crushing hazards while operating the 11 ton bridge crane, serial number 04-3722, which had not received an annual inspection since March 31, 2010.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: Welding Shop: On November 26, 2013 a second follow-up inspection took place after abatement information was not submitted for the Amended Notice of Unsafe and Unhealthful Working Conditions received on January 18, 2013. The space in front of electrical panel PP2 was blocked by a stool, torch cutting table, and an oxygen/acetylene cart. THE U.S. ARMY RESERVE -AMSA-13/63rd RSC WAS PREVIOUSLY CITED FOR A VIOATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 108372, NOTICE NUMBER 01, ITEM NUMBER 02, ISSUED ON FEBRUARY 3, 2012, AND AFFIRMED AS A FINAL ORDER ON MAY 21, 2012, WITH RESPECT TO A WORKPLACE LOCATED AT 1480 RAILROAD AVENUE, NOVATO, CA 94592.
