The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    a. Production Department - On or about November 26, 2013, the employer did not have the points of operations completely guarded, in that the mill #1,  Yoder M, 1 1/2 ERW Tube Mill, was not guarded from inadvertent contact and did not prevent an employee from reaching into the points of operation, thereby exposing employees to crushing and amputation type injuries.      b. Production Department - On or about November 26, 2013, the employer did not have the points of operations completely guarded, in that the mill #2,  Yoder M,  2 ERW Tube Mill, was not guarded from inadvertent contact and did not prevent an employee from reaching into the points of operation, thereby exposing employees to crushing and amputation type injuries.      c.  Production Department - On or about November 26, 2013, the employer did not have the points of operations completely guarded, in that the mill #5,  T.H. Lemont WU25 Mill, was not guarded from inadvertent contact and did not prevent an employee from reaching into the points of operation, thereby exposing employees to crushing and amputation type injuries.      d. Production Department - On or about November 26, 2013, the employer did not have the points of operations completely guarded, in that the mill #6,  OTO Mill, was not guarded from inadvertent contact and did not prevent an employee from reaching into the points of operation, thereby exposing employees to crushing and amputation type injuries.      e.  Production Department - On or about November 26, 2013, the employer did not have the points of operations completely guarded, in that the cutoff /re-cut,  Yoder P-40, was not guarded in that it did not prevent an employee from reaching into the equipment, thereby exposing employees to the hazards associated with points of operations.  f. Removed during Informal Conference  g.  Production Department - On or about November 26, 2013, the employer did not have the points of operations completely guarded, in that the cutoff /re-cut,  Alpha MDLarf-12-48 83-769 84-786, serial number 141, was not guarded in that it did not prevent an employee from reaching into the equipment, thereby exposing employees to the hazards associated with points of operations.    h. 1910.147(c)(6)(i):  The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  Throughout facility - On or about November 26, 2013, the employer did not provide effective periodic inspection to authorized employees in accordance with the aforementioned standard, to include all of the elements in (A) through (D); thereby exposing employees to the hazards associated with unexpected re-energization of equipment.  T & B Tube Company, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147 (c)(6)(i), which was contained in OSHA inspection number 315157768, citation number 1, item number 1, and was affirmed as a final order on July 18, 2011, with respect to a workplace located at 15525 South LaSalle, South Holland, Illinois 60473.
