29 CFR 1910.178(l)(4)(ii)(E): Refresher training was not provided to forklift operators after a condition changed that affected safe operation of the truck:    Warehouse Areas: The employer did not provide adequate refresher training after additional racks were added which affected the travel routes and safe operation of the forklifts.  An employee operating a Crown RR5200 Series Forklift (Serial Number 1A256616) in the Dry Room Warehouse was seriously injured when he turned into a former travel route and cut-through that had been replaced with additional racking.  The collision caused both the employee and the forklift to fall to the ground.
