29 CFR 1910.178(l)(4)(ii)(E): Refresher training was not provided to forklift operators after a condition changed that affected safe operation of the truck: Warehouse Areas: The employer did not provide adequate refresher training after additional racks were added which affected the travel routes and safe operation of the forklifts. An employee operating a Crown RR5200 Series Forklift (Serial Number 1A256616) in the Dry Room Warehouse was seriously injured when he turned into a former travel route and cut-through that had been replaced with additional racking. The collision caused both the employee and the forklift to fall to the ground.

29 CFR 1910.178(n)(6): Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel: Warehouse Areas: Employees were not required to look in the path of travel or keep an unobstructed view while operating forklifts. Employees were allowed to wear hooded sweatshirts and other clothing items that obstructed their view. An employee was seriously injured when the powered industrial truck he was operating collided into the racking system.
