JOBSITE:  The employer did not ensure through visual observation, specific instructions and other means that the loads of plywood were hooked, unhooked and guided by employees were rigged properly to prevent unintentional displacement.    On or about 11/14/13, a load of plywood rigged by unqualified personnel, fell onto the floor joists at the attic level of a residential house causing an employee to fall through the broken floor joists.  The employee sustained broken bones to his left orbital eye bone, left elbow arm, left leg, and his nose.

The employee sustained broken bones to his left orbital bone, arm and leg and his nose.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     JOBSITE:  The employees performing framing work of a residential house under construction at heights between 10 ft 1 3/4 inches to 19 ft 9 inches were not protected by a guardrail system, safety net system, personal fall arrest system or other equivalent means.    On or about 11/14/13, while performing framing work an employee gave signals for proper setting of a hoisted load of plywood, the load fell and cracked the floor joists and the employee fell 19 feet 9 inches to the ground level and sustained serious fractures to his left elbow, left ankle and left orbital eye bone.    Patric's Carpentry, LLC was previously cited for a violation of this occupational safety and health standard {29 CFR 1926.501(b)(13)}, which was contained in OSHA inspection number 313739187, citation number 01, item 003a, and was affirmed as a final order on 3/18/2010, with respect to a workplace located at 84 Juniper Road, New Canaan, CT 06840.
