Location: 701 -706 Farms Drive Burlington, MA, South West side of building     On or about 11/19/13, the following but not limited to aluma-pole pump jack scaffold platforms overhung past the upright more than 8-feet exposing the workers to fall hazards of more than 14-feet;    1.  #1 aluma-pole pump jack scaffold platform overhung past the upright more than 8-feet.  2.  #2 aluma-pole pump jack scaffold platform overhung past the upright more than 3-feet.
