29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: On or about 11/20/13 at a commercial construction site located at 8630 Transit Road in East Amherst, New York; employees installing shingles on a two story building were using a ladder jack scaffold with a platform height of approximately 18 feet. Employees on the scaffold did not have a personal fall arrest system. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 11/20/13 at a roof repair project located at 8630 Transit Road, East Amherst, New York; employees were installing shingles on a two story commercial building. The roof had varying pitches up to a 12 in 12 pitch and there were no fall protection systems in use. NO ABATEMENT CERTIFICATION REQUIRED
