29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe: (a) Back side, new townhouse construction located at 1213 Poston Place, Smyrna, GA 30080. Employee was exposed to amputations while operating a Makita (Model 5007F) 7 1/4-inch Circular Saw without an operating lower guard as a shim was used to prevent the lower guard from retracting below the base plate after a cut was made.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. (a) Back side, new townhouse construction located at 1213 Poston Place, Smyrna, GA 30080. Employees were exposed to fall hazards while installing Hardie-Plank Lap Siding on a pump jack scaffold platform located at heights of approximately 16-feet, 6-inches from the lower level without a fall protection system in place.
