29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     (a) Roof, new residential construction located at 2694 Hughes Street, Smyrna, GA 30080. Employees installing 30# Roofing Felt on a 8/12 pitched roof at heights of approximately 13-feet, 8-inches to 14-feet, 5-inches were exposed to falls when a fall protection system was not provided.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    (a) Front roof, new residential construction located at 2694 Hughes Street, Smyrna, GA 30080. Employees were exposed to fall hazards as the ladder provided to access the roof located at heights of approximately 14-feet, 5-inches did not extend at least 3 feet past the working surface and was not secured to the structure to prevent accidental movement while in use.
