29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety net system. On or about November 27, 2013, on the east side of the commercial construction site, employees were exposed to unprotected fall hazards of approximately 9 feet while engaged in roofing activities.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. On or about November 27, 2013, on the east side of the commercial building, employees utilized an extension ladder to access the roof and the side rails did not extend 3 feet above the landing exposing employees to a fall hazard of approximately 9 feet to the ground below.
