29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  Worksite: The employees who were installing asphalt roofing shingles with pneumatic nail guns were not wearing eye protection.   Ray Hydar dba Mac Exteriors was previously cited for a violation of an equivalent occupational safety and health standard 29 CFR 1926.102(a)(1) which was contained in OSHA inspection number 904526, citation number 1, item number 1, issued on 06/11/2013 with respect to a workplace located at 1298 Hartford Turnpike, New Haven, CT, and became a final order on 08/03/2013.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  Worksite: The employees who were working on the 5 in 12 pitch roof up to approximately 35 feet above the ground were not protected from fall hazards by the use of guardrails, safety nets or personal fall arrest systems.  Ray Hydar dba Mac Exteriors was previously cited for a violation of an equivalent occupational safety and health standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 904526, citation number 1, item number 2a, issued on 06/11/2013 with respect to a workplace located at 1298 Hartford Turnpike, New Haven, CT, and became a final order on 08/03/2013.
