29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:   a) On or about December 2, 2013- at a jobsite located at 4100 W. Washington Blvd, Chicago, IL - an employee working beneath a two point suspended scaffold was not protected by the use of a hard hat, thereby exposing the employee to struck by hazards.

29 CFR 1926.451(d)(16): In addition to the normal operating brake, suspension scaffold power-operated hoists and manually operated hoists shall have a braking device or locking pawl which engages automatically when a hoist makes either of the following uncontrolled movements: an instantaneous change in momentum or an accelerated overspeed:   a) On or about December 2, 2013- at a jobsite located at 4100 W. Washington Blvd, Chicago, IL - an employee was performing tuckpointing operations from a two point suspended scaffold approximately thirty (30) feet above the ground, that was not equipped with a braking device or locking pawl which engages automatically following an uncontrolled movement, thereby exposing the employee to fall hazards.

Washington Blvd, Chicago, IL - an employee was performing tuckpointing operations from a two point suspended scaffold that did not require a positive crank force to descend, thereby exposing the employee to falls hazards of approximately thirty (30) feet.

29 CFR 1926.451(e)(1):  When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used:   a) On or about December 2, 2013- at a jobsite located at 4100 W. Washington Blvd, Chicago, IL - an employee was performing tuckpointing operations from a two point suspended scaffold did not have a ladder to safely access the scaffolding, thereby exposing the employee to falls hazards of approximately thirty (30) feet.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   a) On or about December 2, 2013- at a jobsite located at 4100 W. Washington Blvd, Chicago, IL - employees performing tuckpointing operations on the roof were exposed to fall hazards of approximately thirty five (35) feet above the ground created by the presence of unprotected sides and edges.
