29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent): Alexander Lumber Co. had employees exposed to 10 ft. fall hazards without fall protection from the work platform surrounding the work station of a crane located on a Sterling truck.

29 CFR 1910.27(d)(3): "Ladder extensions." The side rails of through or side-step ladder extensions did not extend 3 1/2 feet above parapets and landings: Alexander Lumber Co. had employees exposed to 10 ft. fall hazards, while accessing and exiting the Sterling truck mounted crane work platform, due to the lack of ladder extensions extending a minimum of 3.5 ft. above the work platform.
