(a)  Astronic Industries Trafoline Machine - On or about November 5, 2013 an employee was exposed to struck-by and crushing hazards from inadvertent operation while he adjusted core laminations inside an automatic stacker. The employee was not protected from the hazard by removing and retaining the control on/off key.
