OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushing hazards due to not retaining the control on/off key while working inside a stacker machine. (a) Astronic Industries Trafoline Machine - On or about November 5, 2013 an employee was exposed to struck-by and crushing hazards from inadvertent operation while he adjusted core laminations inside an automatic stacker. The employee was not protected from the hazard by removing and retaining the control on/off key.

29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed: (a) Astronic Trafoline Machine - On or about November 21, 2013 employees were exposed to crushing and struck-by hazards while performing maintenance on the machine. Employees were not protected by their employer certifying that an annual inspection of the machine's lockout/tagout system had been performed.
