29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:  a. At Manchester and North Sappington Road, tree trimming was conducted from an aerial lift; a personal protective equipment hazard assessment had not been certified, that the use of personal protective equipment such as harnesses, hard hats, safety glasses had been evaluated and employees trained on what equpment was required to be worn and under what conditions.
