The insulation was pulled away.

29 CFR 1926.501(b)(13):  Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure:      a. Employees were exposed to a fall hazard up to 12 feet 6 inches while installing sheathing and no fall protection was being used.     b. Employees were exposed to a fall hazard up to 8 feet 4 inches while installing trusses and using a ladder to gain access to the interior top plate and no fall protection was being used.    c. Employees were exposed to a fall hazard up to 10 feet while installing roofing material and accessing the roof at Lot 16, and no fall protection was being used.
