29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:  a) 213 Green Lane, Philadelphia, PA - The employees were working on a fabricated framed scaffold approximately 32 feet and 6 inches high, 5 feet wide and 35 feet long, which was not fully planked or decked on all working levels from the front uprights and the guardrail supports. This hazard exposed the employees to a fall hazard.This violation was observed on or about November 21, 2013.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

This violation was observed on or about November 21, 2013.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. The employees would climb the vertical supports in order to gain access to the working levels of the scaffold, thus exposing the employees to a fall hazard.This violation was observed on or about November 21, 2013.
