29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of Carbon Monoxide listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 50 ppm: a) On or about November 26, 2013, an employee working at in the production area at a CNC machine was overexposed to carbon monoxide at an 8 hour time weighted average of 140.90 ppm, which exceeded the OSHA permissible exposure limit of 50 ppm by 282%. b) On or about November 26, 2013, an employee working at in the production area at a CNC machine was overexposed to carbon monoxide at an 8 hour time weighted average of 324.00 ppm, which exceeded the OSHA permissible exposure limit of 50 ppm by 648%.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: 1.) On or about November 26, 2013 the employer failed to train employees on the elements of a written hazard communication program including the hazards of the chemicals in their work area. Employees were exposed to hazards from carbon monoxide gas from the exhaust of a propane powered forklift used in the facility.
