29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: Employees operating and setting up CNC lathes and mills were exposed to amputation and caught in hazards where the safety interlocks were disabled, by-passed, or missing.
