The terra cotta insulation around the electric lines was damaged and broken off, exposing employees to potential electrical hazards.  Observed on or about 12/3/13.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance paragraph (b) or (c) of 29 CFR 1926.652:  a) Jobsite, 1601 S. 49th St., Philadelphia - Employees walking and operating equipment on the ramp into the excavation in  Type C soil were not protected from cave-ins.  The walls of the ramp were approximately 6 to 8 feet deep and on the side closest to the building they were continuous with the spoil pile which was approximately 10 feet high.  The walls of the ramp and the spoil pile were sloped at an angle steeper than one and one-half horizontal to one vertical (34 degrees from the horizontal).  Observed on or about 12/3/13.  b) Jobsite, 1601 S. 49th St., Philadelphia - An employee walking on planks on top of a trench box in an excavation in Type C soil approximately 25 feet deep, 30 feet wide and 30 feet long, was not protected from cave-ins.  The excavation walls sloped at an angle steeper than one and one-half horizontal to one vertical (34 degrees from the horizontal).  Observed on or about 12/3/13.
