29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: a) Baillie Lumber Co., L.P., Galion, Ohio: On or about December 3, 2013, the employer did not ensure that the Caterpillar V200 lift truck was removed from service when not in safe operating condition, in that the horn was not functioning properly. Employees working around the lift truck were exposed to struck-by and crushing hazards.

The lift truck had a faulty horn, which exposed employees working around the lift to struck-by and crushing hazards.
