29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) At the corner of 1824 E. 1st Street, Maryville, Missouri, a trench was being dug for the installation of a 16" pipe for a water line. The trench where the exposed employee was working was approximately 6 feet deep, 30 feet long and 32 inches wide and not provided with a ladder for access or egress. ABATEMENT CERTIFICATION IS NOT REQUIRED FOR THIS ITEM.

29 CFR 1926.652(a)(1): 29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): (a) At the corner of 1824 E. 1st Street, Maryville, Missouri, a trench approximately 6 feet deep, 30 feet long and 32 inches wide was being dug for a 16"pipe for a water line. One employee was exposed to cave-in hazards when the employer did not bench or provide protective shoring systems in the trench. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19
