29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.  At a jobsite in Carol Stream, Illinois, the erecting, moving, dismantling, and altering of metal frame scaffolds was not done under the supervision and direction of a competent person.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  At a jobsite in Carol Stream, Illinois an employee working from the platform of a metal frame scaffolding system approximately 12 feet above grade was not using any method of fall protection.  There is no abatement certification or documentation required for this item.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable:  At a jobsite in Carol Stream, Illinois, an employee was not trained by a person that was qualified in the recognition of hazards while working from a metal frame scaffolding system.  Abatement certification is required of this item in accordance with the requirments of 29 CFR 1903.19(c).
